To bridge gaps in healthcare access for underserved, newcomer, and aging populations by delivering patient-centred, virtual-first medical care that is accessible, coordinated, and culturally responsive.

Our Care Model

We integrate the expertise of licensed physicians, physician assistants, and nurse practitioners with personalized in-home support delivered through CedarCare Home Health. Our model emphasizes continuity of care, chronic disease management, and timely access to medical services. Services are IFHP-eligible and offered in multiple languages to meet the needs of diverse communities.
